Teen Costume Dance
OHEN is proud to sponsor and announce its 2nd Annual Teen Costume Dance!
DATE: Sunday, October 26, 2008
TIME: 7:00-11:00 p.m.
PRICE: $15 per person ($20 at the door)
Ticket price includes entrance to the dance and refreshments. Please use the order form below and mail it in as soon as possible to order your tickets. To ensure delivery of your tickets, orders must be postmarked by Thursday, October 16, 2008.
WHERE: The Northstar Ballroom, 635 North Killingsworth Court in Portland. Located in North Portland, near the PCC Campus with easy I-5 access, just 10 minutes from downtown & 5 minutes from Lloyd Center. Visit the

WHO: The Teen Costume Dance is for homeschoolers 13 years old through high school age. Dates are not required. If you do bring a date or a friend, your
date does not have to be a homeschooler (but still at least 13 years old). One person in a "couple" needs to be a homeschooler.
MUSIC: A variety of dance music will be provided by DJ Zeke! (The music will be profanity free.)
COSTUMES: There will be prizes for costumes in the following 5 categories: Most creative, Most scary (not gory), Prettiest, Craziest, Best Make-up. (Please avoid wearing costumes that are graphic, gross or revealing.) Thanks!
PARKING: There should be plenty of street parking on a Sunday evening and also, PCC has allowed us to use their parking lot that is adjacent to the ballroom.
PHOTOGRAPHY: If you would like a professional photo of you & your friends, Black Box Photography will be there to capture a great shot for only $5 per
picture -- and you'll get to take the picture home with you!
GUIDELINES: To make the dance comfortable and safe for everyone, the following will not be allowed:
- re-entry privileges to the dance
- horse play
- running
- making out
- inappropriate dancing
SCHOLARSHIPS: Scholarships may be available for those who require additional financial assistance. Please email Diana with your request at mom@blackboxphotography.com.
CHAPERONES: There will be plenty of chaperones. If you are interested in being a chaperone, please email Diana at mom@blackboxphotography.com.
